What to Expect When You Visit One Family Practice
To ensure you receive the best care possible, here’s what you need to know and prepare for your visit:
Before Your First Visit:
Medical Records: Please bring a copy of your medical records with you to your first appointment. This will help me understand your medical history and provide personalized care.
Prescriptions and Supplements: Bring all the prescriptions and supplements you are currently taking. This will assist in medication reconciliation and ensure safe and effective treatment.
Immunization Records: Please bring a copy of your immunization records to keep your file up-to-date.
Referral Information: If you are seeking a referral, bring the necessary details, including the facility or specialist's name, phone number, fax, and address.
During Your Visit:
Weigh-In and Vital Signs: Your weight and vital signs, including blood pressure, pulse, oxygenation, pain level, heart rate, and respirations, will be taken at each visit to monitor your health status.
Consent Forms: You will be asked to sign consent forms as needed for treatments and procedures.
Insurance Verification: Please bring your insurance card to every visit for verification purposes.
Discuss Your Concerns: Bring a list of the top two issues you wish to discuss, along with any questions you have. This helps prioritize your concerns and make the most of our time together.
Limit of Two Issues: Due to time constraints, only two issues will be addressed per visit. Future appointments will be needed for additional concerns or treatments.
Future Visits and Paperwork:
Additional Appointments: A separate appointment will be scheduled for each additional ailment or need beyond the two discussed issues.
Paperwork Visits: If you need paperwork filled out, this will be considered one visit. During this appointment, only paperwork will be addressed, and no medication refills will be provided.
